Summary and Background Information: Between February 2015 and January 2019, the State of Colorado will receive up to $65 million from the Center for Medicare and Medicaid Innovation (CMMI) to implement and test its proposed model for health care innovation.The Colorado State Innovation Model , entitled “The Colorado Framework,” creates a system of clinic-based and public health supports to spur innovation, setting the stage for a sweeping transformation that will help accelerate the state's progress toward the Triple Aim of lower costs, better care and improved population health. It will also allow Colorado to reach the goal of becoming the healthiest state.

 

Community Reach Center and Salud Family Health Center have worked together for several years in providing integrated health care in Salud clinics.  Together, they offer a highly developed system of care for the community of Commerce City through the unique Integrated Health Home Program, which offers a wide range of resources that can be combined to offer a full complement of services to patients. To expand this work, Community Reach Center was one of only four mental health centers to receive a Colorado State Innovation Model (SIM) grant valued at $840,000 for four years to better understand the true cost of integrated care as well as alternative payment methods.

 

Community Reach Center will provide an update on the grant funded project for city council members and encourage individuals to attend the May 17th open house from noon - 1:30 p.m.
